Form A : Church - Line of Credit or Amortized Loan
Form R: Church  - Renewal or Conversion of an existing loan
Form M: James Muzzy Loan - Equity Share and Insurance Loans
Form V: Renewal of an existing James Muzzy Loan
Form B: Presbytery - Line of Credit or Amortized Loan
Form O: Related Organization - Line of Credit or Amortized Loan
